Oakland EZ Voucher Checklist
This checklist must accompany each voucher request
and must be signed by the person completing the form.
For your convenience, we have provided a PDF form that
can be completed with Adobe Reader. Please note that
the form includes some references on the right side
after each section. These references coincide with the
information regarding qualification categories and
acceptable documentation - item # 3 below. Please
review the appropriate category information prior to
submitting your voucher request.
Employee Template
Excel template that provides specific employee data
regarding voucher requests. A separate template must be
provide for each business. The template requires three
identification codes: (1) Agent; (2) Business and (3)
Location. The identification codes and the template are
obtained through the online registration process. The
online system can be accessed by

Oakland Qualified Category Information
Matrix includes information about qualification
categories and acceptable documentation. The numbering
of the categories coincide with the reference numbers
provided on the Oakland EZ Voucher Checklist. The list
of acceptable documentation is not intended to be all
inclusive. However, the submitter should receive prior
permission before submitting documentation outside of
the list of acceptable documents provided. Any request
to accept information outside of this list should be
sent through the
Oakland Online Vouchering System. You would sign in
through the agent module, enter the message center area
and direct your question/request to the enterprise zone
coordinator. Please note that any voucher request sent
with documentation outside of this acceptable list will
experience a delay in processing.
